INSTRUCTION
on rule of issuance of de-registration cards for voting in presidential elections of the Republic of Azerbaijan

1. Requirements concerning the for providing with de-registration cards for voting during elections shall be regulated by Article 101 of Election Code of the Republic of Azerbaijan (hereinafter referred to as Election Code) and this Instruction.
2. In the case when a voter is outside of the relevant precinct in which he/she has been included in the voters’ list or votes in another elections (referenda) precinct on the voting day, then he/she can obtain voting card from the relevant election (referenda) commission (Appendix # 1 made to Instruction).
3. Constituency Election Commissions (Con.EC) shall provide the voter with voting card 45-25 days prior to the voting day and Precinct Election Commission (PEC) shall provide the voter with this card 24-3 days prior to the voting day.
4. The voting cards for voting shall be issued by the relevant election commission to the voter according to his/her application, or to the voter’s representative, based on a power of attorney verified by a notary office. The voter’s application should indicate the reasons why the voter could not get the voting card (Appendixes # 2, 2a, 2b and 2 c made to Instruction). According to this application, the voter shall be provided with voting card.
5. The precinct election commission should apply to the constituency election commission within two days and clarify whether the voter has obtained a voting card before or not. It shall be prohibited to provide a voter with more than one voting card.
6. The constituency election commission shall make a register of issuance of cards for voting. The voter’s surname, name, patronymic, date of birth and address should be indicated in the register (Appendix # 3 made to Instruction). The constituency election commission shall send the voters lists together with the verified extracts from the mentioned register to the precinct election commission, 25 days before the Election Day. Based on such extracts, each precinct election commission should make relevant notes in the voters’ lists.
7. When receiving a  voting card  from the constituency election commission, a voter or his/her authorized representative shall sign the register and indicate the series and number of his/her ID or substitute document. In this case the cut-off corner of the voting card shall be attached to the register.
8. The de-registration card for voting issued to the voter by the constituency election commission shall be confirmed by the stamp of that constituency and signed by a member of the constituency election commission.
9. When receiving a  voting card  from the precinct election commission, a voter or his/her authorized representative shall sign and indicate the series and number of his/her ID or substitute document. In this case the cut-off part of the voting card shall be attached to the voters’ list.
10. The voting card provided to the voter shall be signed by the member of the election (referenda) commission and approved by the stamp of this commission.
11. Immediately after providing the voter with voting card, the relevant election (referenda) commission shall inform the Constituency Election (referenda) Commission on this. And the Con.EC shall submit this information to “Sechkilar” Information Center of CEC once after every two days.
