LIST
of document submitted to the Constituency Election Commission by observers who are to be registered (Article 40.7 of the Election Code of the Republic of Azerbaijan)

1. A citizen shall submit the following documents to take part as an observer on his/her own initiative:
- application of the observer (the application should contain surname, name, patronymic, date of birth, address of residence and serial number of ID card or its substitute document of the observer; two photos (3X4) of the observer should be attached).
2. The registered candidate shall submit the following documents to appoint observer:
a) appeal to the Constituency Election Commission (Con.EC) (the appeal should contain the list of observers);
b) application on consent of the observer (the application should contain surname, name, patronymic, date of birth, address of residence, serial number of ID card or its substitute document of the observer; two photos (3X4) of the observer should be attached).
3. The following documents shall be submitted for the registration of observers appointed by political parties, blocks of political parties and non-governmental organizations engaged in the field of elections:
a) appeal to the Con.EC (the appeal should contain the legal address of the organization);
b) copy of the registration certificate of the organization approved in the notary order (excluding political parties and non-governmental organizations whose documents have been submitted to the Central Election Commission to participate in elections and appoint observers);
c) copy of the Charter of the organization approved in the notary order (excluding political parties and non-governmental organizations whose documents have been submitted to the Central Election Commission to appoint observers);
d) session minutes on the appointment of the observation;
e) list of the appointed observers (the list should contain surname, name, patronymic, date of birth, address of residence and serial number of ID card or its substitute document of the observer);
f) application on consent of each appointed observer (two photos (3X4) of the observer should be attached to the application);
g) besides the above-mentioned documents non-governmental organizations shall submit official references on the share of foreign, physical and legal entities in the charter capital that should not exceed 30%, not getting grant by legal entities of the Republic of Azerbaijan and their function in the field of elections according to Article 2.4 of the Law of the Republic of Azerbaijan “On Non-Governmental Organizations”.
