C O M M E N T
on rules for completing signature sheets
1. Signature
sheets shall be prepared by the Central Election
Commission in the form defined by the Appendices
#2 and 2 ”A” specified by the Election Code
of the Republic of Azerbaijan.
1.1. While completing the signature sheet the
upper left side of each signature sheet shall
contain name and the number of the constituency
(election area) where signatures are collected.
1.2. The column for the surname, name, patronymic
name of the candidate of each signature sheet
shall contain date of birth, main working or
service place, or current position ( if he/she
does not have it, kind of occupation), address
of residence.
1.3. IDPs shall indicate the address of their
current residence in the column for the address
of residence while putting their signatures.
In the case of not having the identification
card or document that substitutes it of the
internal displaced in the column of signature
sheet for serial number of passport or document
that substitutes it, IDP cards prepared in the
form defined by the State Committee on IDP issues
shall be used and the number of the IDP card
shall be noted by indicating the country (city)
in the column.
1.4. If the address of residence indicated in
the ID card has been changed, then in the column
of signature sheet for the address of residence
first the address of previously indicated in
the ID card, then in the brackets the changed
new number must be indicated.
2. The reverse side of the signature sheet shall
contain the address of residence including the
information noted on the signature sheet of
the person collecting signatures.
2.1. If the candidate to presidency was nominated
by voter initiative group or self-nominated,
then at the end of the signature sheet the column
for the surname, name, patronymic name, personal
signature and date of signature of the candidate
shall be approved by being signed by the candidate
to presidency himself.
2.2. In the case of nominating the candidate
by political party, blocs of political parties,
after collecting signatures the authorized representative
of political parties, blocs of political parties
puts his signature in the column for the candidate
to presidency by indicating his surname, name,
patronymic name and date of signature.
3. Political parties, blocs of political parties
or voter initiative groups must collect no less
than forty five thousand voters signatures in
support of the candidates they nominated. At
least 50 signatures must be collected from the
area of each constituency not less than 60.
3.1. Signature sheets submitted to the Central
Election Commission must be numbered and made
up into pages in the form of a folder.
